چکیده

A 46-year-old woman with moderate lens subluxation and Marfan ́s syndrome underwent a phacoemulsification on the right eye with a sutureless Henderson Capsular Tension Ring (HCTR) implantation. The surgical procedure and the postoperative follow up period were uneventful, with a best corrected visual acuity (BCVA) of 0 (LogMAR). Seven months after surgery the patient presented an inferomedial displacement of the HCTR due to contraction of the capsular bag, displacing the ring over the inferior haptic of the IOL that was pulled posteriorly beneath the equatorial area but remained centered. No further treatment was prescribed. The HCTR did not maintain an adequate capsular bag expansion and failed to redistribute tension from non-affected zonules over the areas of zonulysis. This case report raises the hypothesis that modified CTR ́s stiffness should be evaluated as an independent factor in the selection of a proper device for each patient.

Central cortical cleanup and zonular deficiency

BACKGROUND Complete removal of the cortex has been advocated to prevent posterior capsular opacification but carries the risk of zonular dehiscence, hence there is a need for a safe maximal cortical cleanup technique in eyes with severe diffuse zonulopathy in subjects above age 90. METHODS We used bimanual central cortical cleaning by elevating central fibers and aspirating them toward the pe...

Capsular Tension Rings

Cataract surgery in an eye with absent or weak zonules presents unique challenges for the anterior segment surgeon. With the introduction of the capsular tension ring (CTR), and its several modified versions, the ability to perform safe cataract extraction with the implantation of a stable and well-centered intraocular lens (IOL) within the capsular bag has increased substantially. In order to ...

Capsular tension rings and related devices: current concepts.

PURPOSE OF REVIEW To discuss current designs, indications, contraindications and controversies pertaining to capsular tension devices. RECENT FINDINGS Capsular tension rings and other newer endocapsular support devices have become increasingly important in the management of zonular weakness during cataract extraction. They have been found to improve both intraoperative support during phacoemu...

Outcomes of modified capsular tension ring versus sulcus-fixated foldable intraocular lens in subluxated lens secondary to Marfan syndrome

Purpose: To determine the outcomes of implantation of a modified Capsular Tension Ring (CTR) and a Posterior Chamber Intraocular Lens (PCIOL) in patients with subluxated lens secondary to Marfan Syndrome (MFS). Materials and methods: A total of 21 eyes of 12 patients with subluxated lens secondary to MFS undergoing Posterior Chamber Intraocular Lens (PCIOL) from March 2004 to February 2005 were...

The effect of capsular tension ring on posterior capsular opacity in cataract surgery.

This study was performed to evaluate the efficacy and safety of the capsular tension ring on posterior capsular opacity in comparison with cases undergoing intraocular lens (IOL) implantation alone. We analyzed 127 eyes which had undergone cataract surgery, including capsular tension ring insertion, along with 127 eyes which had undergone IOL implantation alone by the same surgeon from Septembe...
